How Do I Create a New Observation?

This article will cover how to add observations about a patient. Observations may include either Physical, Pathology or a variety of other observations.

Follow these steps to create a New Observation:

1. Open the patient record.
2. Make sure you are in the "Clinical" tab
3. Make sure you are in the "Observations" section
   
4. Click Add Observation to reveal a drop-list of observations
   
5. Click Pediatric Vitals (example) to open the observation form
   
6. Complete the form with all the required details
7. You do not have to fill in every observation field - just those that are relevant to the observation
8. The Add to Today's Notes tick-box is always checked, and cannot be unchecked
9. The observation will automatically be added to the consultation notes
10. Click Save to complete.

The new observation has now been created.
