This step-by-step guide walks you through the process of uploading, sending, and unpublishing consent forms in Engage. It covers how to publish signed consent forms for record-keeping, send them to patients for completion, and manage their status, including how to unpublish consent forms if necessary.

π Upload and Publish Signed Consent to Engage
1. Navigate to the Consent tab within the patient's record

2. Click on New Consent.

3. Complete all mandatory fields. Do not select the Patient to Complete in Engage checkbox.

βΉοΈ Since you are uploading a signed consent form for record-keeping purposes, do not tick the Patient to Complete in Engage checkbox.
The workflow for requesting patient completion via Engage is outlined in Step 7.
4. Click on Save to confirm.

βΉοΈ This action will automatically publish the uploaded form to Engage for the patient to view.
5. A pop-up will appear to indicate that the Consent has been successfully created.

6. Click the βiβ (information) icon to confirm the consent form has been automatically published to Engage.

π Sending Consent Form for Patient to Complete via Engage
7. Navigate to the Consent tab within the patient's record

8. Click on New Consent.

9. Tick the Patient to Complete in Engage checkbox to send the consent form to the patient for completion via their Engage portal.

10. Complete all mandatory fields.
βΉοΈ The Status will auto-populate to Proposed.
11. Click on Save to confirm.

βΉοΈ This action will automatically publish the consent form to Engage, where the patient will be prompted to either approve or deny it.
12. A pop-up will appear to indicate that the Consent has been successfully created.

13. Click the βiβ (information) icon to confirm the consent form has been automatically published to Engage.

14. In the Consent grid, the Provision Status will update to indicate that the form has been sent to the patient via Engage.
15. Once the patient has actioned this in Engage the Provision status will automatically update to Permit or Deny.

βΉοΈ The Provision Status in the Consent grid reflects the patientβs response to the consent form sent via Engage:
- Permit β The patient has approved the consent form.
- Deny β The patient has declined the consent form.
16. Click the βiβ (information) icon will indicate the patient has actioned the consent form in Engage.

π Unpublishing a Consent form from Engage
If a consent form is Published to Engage in error you will need to update the Status.
17. Navigate to the Consent tab within the patient's record

18. Tick the checkbox next to the relevant consent form listed in the grid.

19. Click on Edit.

20. Click the drop-down arrow in the Status field to view and select the appropriate option.

21. Select the appropriate option from the list.

βΉοΈ A consent form is considered unpublished if its status is set to one of the following:
- Draft
- Inactive
- Entered in Error
Selecting any of these statuses will remove the consent form from Engage, making it no longer visible to the patient.
22. Click on Save to confirm.

23. Click the βiβ (information) icon to confirm that the consent form is no longer published to Engage.
