Recording and reviewing patient observations is a key part of ongoing care. In MediRecords, observations can include physical measurements, pathology results, and a variety of other clinical data points. These records are stored in the patient’s clinical record, where they can be viewed in detail or compared visually over time, helping providers to identify changes, trends, and potential concerns.
The Observations feature also makes it simple to add new vitals during a consultation, edit existing entries, or delete outdated information. Clinicians can print records, generate percentile charts for paediatric patients, and even graph results against previous recordings for clearer insights. By keeping observations up to date, practices can support more informed decision-making and provide safer, more consistent care.

Viewing Vitals
The Observations page displays the complete history of observations of a patient which is viewable as individual records or plotted on a timeline graph.
Open a Patient record
Go to Clinical Section
Go to Observations

Select the Category and Sub-category (in the below example, Physical > Pulse) and the observations will be displayed in the table on the right-hand side.
To sort by each column in the observation window, click on the relevant heading at the top of the table. The below example is sorted by Date, denoted by an upwards arrow.
Graphing Results over Time
Timeline Graph function is helpful to compare Vitals recordings taken for the same patient, over time. To view the graph
Open patient record
Go to Clinical tab
Go to Observations section
Click the Observation Name (for example temperature)
All observations for this sub-category will be displayed in the observation grid.
Click View > Timeline Graph
Click another Observation Category/ sub-category to display the results on the timeline graph
Adding an Observation
Open patient record
Go to Clinical tab
Go to Observations section

Click Add Observation to reveal a drop-list of observations.
Click on the observation you wish to record to open the observation form. (for e.g. ROM Lower Limb)
Complete the form with all the required details

Click Save to complete.

The observation will now be recorded and available to view by selecting the category (Rom-Lower Limb) and the sub-category (ankle-foot).
Editing an Observation
Open patient record
Go to Clinical tab
Go to Observations section
Select the Category and sub-category the observation belongs to. This will display all the recorded observations in the observations grid.
Select the tick-box next to the observation you wish to edit

Click Edit
The recorded observation will become editable.
Click enter after editing.
Deleting an Observation
Open patient record
Go to Clinical tab
Go to Observations section
Select the Category and Sub-category of the observation you wish to print (Physical and Temperature in the example below)

Click Delete to permanently delete the observation record

Click Yes to acknowledge the observation will be deleted permanently.
The observation result has now been permanently deleted.
Printing an Observation
Open Patient record
Go to Clinical tab
Go to Observations section
Select the Category and Sub-category of the observation you wish to print (Physical and Temperature in the example below)

Click Print to open a print preview
Check to make sure the page is going to the right printer Destination and click Print.
The observation has now been printed.
Percentile Charts
Open patient record
Go to Clinical tab
Go to Observations section
Click the Percentiles tick-box
To view a chart, simply click on the check box next to the observation in the observations grid
Select View

Select Timeline Graph
Percentiles will now be displayed in observations within the patient record.
