The Audiogram clinical template lets you record the results of a pure-tone audiometry test to assess a patient's hearing and identify any level of hearing loss. It shows previous audiogram results alongside current findings for easy comparison, and includes sections for documenting ear examination notes and relevant ENT (ear, nose, and throat) history.

Accessing the Audiogram Template
The Audiogram template can be found within the Observation drop-down above the Today's Notes area, or via the Add Observations button in the Observations grid.
Field Reference
| Field | Description |
|---|---|
| Date | Defaults to today's date. This is a required field. |
| Frequency (Hz) | Records the dB value for each ear at 500, 1000, 1500, 2000, 3000, 4000, 6000, and 8000 Hz. Accepts negative values, with a valid range of -10 dB to 120 dB. |
| Last Frequency Observation | Automatically displays the most recent frequency observation and its date, so you can compare against the current assessment. This field is read-only. |
| Clear from Aural Infection? | Yes/No selection. Selecting Yes automatically sets the Right Ear and Left Ear fields below to Normal. |
| Right Ear / Left Ear | Dropdown of Normal, Abnormal, or Non Testable. |
| Any Previous ENT Problems or Operations? | Yes/No selection. Selecting Yes enables a details text area (up to 500 characters). |
| Any Current ENT Problems? | Yes/No selection. Selecting Yes enables a details text area (up to 500 characters). |
| Comments | Free text, up to 500 characters. |
