Patient discharge and transfer management is the process of formally closing a patient’s admission to a healthcare facility. It involves updating records when a patient leaves the facility, either by being discharged home or transferred to another healthcare service for ongoing care, ensuring accurate documentation and continuity of care.

Transferring a Patient
- Select the patient you want to transfer from the patient list.
- From the Dashboard tab, select the New Letter shortcut in the Quick Actions bar.
- Once the letter opens, complete all required fields, then select Save.
- Once you have completed the discharge summary in the Patient Admission tab, select the Admission Status drop‑down menu, then select Ready to Discharge.
- Fill out all the required fields and select discharge. The patient will now be successfully discharged for the transfer.
Discharging a Patient
- Select the patient you want to discharge from the patient list.
- From the Dashboard tab, select the New Letter shortcut.
- Once the letter opens, complete all required fields before proceeding. Once the letter opens, complete all required fields, then select Save.
- After saving the letter, select Finalise, then you will be able to send it. Please note that sending is only available if the recipient has secure messaging enabled.
- Once you have completed the discharge summary in the Patient Admission tab, select the drop‑down menu next to the Emergency Status field, then select Ready to Discharge.
