This article explains how to connect your practice to My Health Record (MHR) in MediRecords, so providers can securely view and upload information such as pathology results, prescriptions, and discharge summaries. It covers the HI and NASH certificate prerequisites, practice-level configuration, and individual provider setup. This applies to any practice enabling MHR. Before starting, apply for your HI and NASH certificates, both are required before configuration can begin.

Pre-Requisites
Before you can connect to My Health Record, you need to register your practice with Medicare, obtain access to the Health Identifier (HI) Service, and obtain the Health Professional Identifier - Organisation (HPI-O) Number.
Access to the HI Service, which requires both a HI Certificate and a NASH Certificate, is required to access My Health Record.
- You will need to complete the Healthcare Organisation registration form on the My Health Record website. Applications can be completed either online or by mail. Once you have completed registration, you will be provided with an HPI-O Number and a NASH Certificate which needs to be uploaded to your practice profile in MediRecords.
- Obtain a copy of all of your providers' HPI-I Numbers. To use My Health Record, all providers must have an HPI-I Number recorded against their user profile in MediRecords. If the HPI-I Number is not recorded, then that provider will not have access to My Health Record.
Note: If you cannot locate a provider's HPI-I Number, they can contact either HPOS or AHPRA to obtain a copy of their 16-digit HPI-I Number.
Once you have the HI and NASH certificates with their passphrases, the HPI-O Number for your practice along with your providers' HPI-I Numbers, you can configure your practice settings and users to connect to My Health Record.
Part 1A: Practice Configuration - HI Certificates
To begin setting up My Health Record, you will need to upload your Health Identifier PKI Certificate to your practice profile in MediRecords, within the Health Identifiers section of Practice Configuration.
- Go to More.
- Click Settings.
- Click Configuration.
- Click the relevant Practice Name.
- Open the Health Identifier tab.
- Enter your Certificate Passphrase for your HI Certificate (also known as PIC) into the PIC - Personal Identification Code field.
- To upload your HI Certificate, click Select File to open a file explorer window. Navigate to the location of your HI Certificate (only select the one called fac_sign.p12) and select it for upload.
- Once the certificate has been saved, the Certificate Alias Name and the Expiry Date will automatically be populated on the form.
- Click Save.
Part 1B: Practice Configuration - NASH Certificates
You will now need to upload your NASH Certificate to your practice profile in MediRecords, within Practice Configuration in Settings.
- Open the eHealth tab.
- Enter your Certificate Passphrase for your HI Certificate (also known as PIC) into the PIC - Personal Identification Code field.
- To upload your NASH Certificate, click Select File to open a file explorer window.
- Navigate to the location of the certificate (typically it will be stored on a CD, which was provided by Medicare), click on the name of the certificate (called fac_sign.p12) and upload.
- Once the file has been uploaded, the practice's HPI-O Number will automatically be populated to the HPI-O No. field, along with a Certificate Alias Name and Expiry Date, which are all digitally stored on the NASH Certificate.
- Click Save.
Practice configuration for My Health Record is now complete.
Part 2: Provider Configuration
For providers to use My Health Record, they must have their 16-digit HPI-I Number recorded into their profile. If a provider does not have the HPI-I Number recorded against their profile, they will not be able to access My Health Record.
The HPI-I Number is added to the Identifiers section of the user profile, within User Settings.
- Go to More.
- Click Settings.
- Click Subscription Details.
- Click on Users.
- Click the name of the provider you wish to configure.
- Open the Identifiers tab.
- Click HPI-I No. and type in the 16-digit HPI-I Number.
- Click Save to apply the changes.

The provider has now been configured to allow access to My Health Record. Repeat this process for any additional providers, and for any new providers that join the clinic in the future.
Troubleshooting
Certificate upload fails or is rejected
Cause: The wrong file was selected (only the file named exactly fac_sign.p12 should be uploaded, not other files in the certificate package), or the Certificate Passphrase (PIC) was mistyped.
Resolution: Confirm you're selecting the file named fac_sign.p12, and re-enter the passphrase carefully, it's case-sensitive.
Escalate when: The correct file and passphrase are used but the upload still fails.
HPI-O Number, Certificate Alias Name, or Expiry Date don't populate after upload
Cause: The wrong certificate type may have been uploaded (an HI Certificate in the NASH Certificate section, or vice versa), or the certificate file is expired or corrupted.
Resolution: Confirm the correct certificate type was uploaded in each section (HI Certificate in Part 1A, NASH Certificate in Part 1B), and check the certificate's expiry date.
Escalate when: The correct certificate type is confirmed but the fields still don't populate.
A provider can't access My Health Record after setup
Cause: Their HPI-I Number is missing, or was entered incorrectly, against their user profile.
Resolution: Check Identifiers under that provider's User Settings and confirm the 16-digit HPI-I Number is present and correctly typed.
Escalate when: The HPI-I Number is confirmed correct but the provider still can't access My Health Record.
