Adding consult notes in MediRecords is a core part of documenting and managing patient care. Today’s Notes provides a flexible and efficient way to capture consultation details - whether documenting a current encounter, adding information from a recent visit, or recording historical consultations.
With features such as SOAP formatting, consult timers, autosave, and the automatic insertion of clinical data, MediRecords streamlines note‑taking so clinicians can focus on patient care rather than typing. Notes support formatting tools, shortcuts, templates, and attachments, helping you create accurate, professional documentation that is easy for the care team to review.

Opening Today’s Notes
To access Today’s Notes and begin documenting the consultation:
- Open the patient record, go to the Clinical tab, and select Today’s Notes.
Consult Preferences
You can customise the behaviour and layout of your consultation notes to match your workflow.

Click the Person icon in the top-right corner, select Preferences, and adjust your consult settings as desired.
SOAP Today’s Note
When SOAP formatting is disabled:
- Any information you add or insert appears wherever your cursor is positioned.
When SOAP formatting is enabled:
- Today’s Notes are automatically divided into:
Subjective
Objective
Assessment
Plan - Clinical items entered through widgets (e.g., Observations, Diagnosis) are placed in the appropriate SOAP section, regardless of cursor location.
To enable or disable SOAP:
- Tick or untick SOAP Today’s Note in your Consult Preferences → click Save.
✅ Tip: Notes are inserted in this order when applicable:
Reason for Visit, Observation, Examination, Procedure, Clinical Drawing, Diagnosis, Shortcuts, New EPC, Cases, Clinical Template, Pathology Request, Radiology Request, Prescription, Immunisation, Extended Care, Patient Education.Default Consultation Type
If you commonly use the same consultation type, you can save time by setting a default.

In Preferences under General, open Default Consult Type, pick your preferred type, and click Save.

Consultation Timer
The timer can start automatically or manually.
- Automatic Start: If enabled, the timer begins the moment you enter Today’s Notes.
- Manual Start: If disabled, you can start the timer yourself.

In Preferences under General, tick the Start Consultation Timer, and click Save.
Consultation Note Behaviour
For Automatic Consultation Finalisation, if you do not manually click Finish, MediRecords will automatically finalise the consultation:
- Consults started before 10 PM AEST are finalised around midnight the same day
- Consults started after 10 PM AEST are finalised around midnight the next day
⚠️ Warning: Once a consultation is finalised, it cannot be reopened.
You may still add an amendment, but the original note cannot be edited.Automatic Insertion of Clinical Data
Clinical activities documented during the consultation automatically appear in Today’s Notes.
This includes:
- Reason for Visit
- Observations
- Examinations
- Procedures
- Clinical Drawings
- Diagnoses
- Reviews & Health Assessments
- Allergies & Reactions
- Medical History
- Medications
- Immunisations
- Investigations
- Correspondence
- Obstetrics
- Cervical Screening Results
ℹ️ Note: If a clinical item is created from the Patient Dashboard without entering the Clinical section first, it will not be added to Today’s Notes.AutoSave
MediRecords automatically saves your note whenever new information is detected.
- A saving indicator appears in the corner while the system saves
- You may continue typing during AutoSave
- You can also click Save at any time
Consultation Date, Time, Type & Room
You can modify key consultation details above the Today’s Notes:
- Date: Click the date → choose from the calendar
- Time: Click the clock icon → select a time
- Type: Click Type → choose from the dropdown

Room: Click Room → choose from available rooms
(Rooms must be created under Practice Settings before they appear here)
Starting and Stopping the Consult Timer
- To pause: Click Pause
- To resume: Click Resume
Finishing a Consultation
Click Finish to finalise the consultation.
If you return to Today’s Notes later on the same day, MediRecords will ask:
- Continue Consultation: reopen the existing note (until midnight AEST)
- Start New Consultation: finalises the previous note and creates a new one
Reason for Visit
- Click Reason
- Use the search field to find the condition
- Select from the dropdown or type manually
- Complete the form → click Save
Diagnosis
- Click Diagnosis

Search or enter the diagnosis, complete the fields, and then click Save.
Formatting the Consult Note
You can use formatting tools, clinical templates, and shortcuts to streamline your documentation.
Formatting options include:
- Headings
- Bold, italics, underline
- Left/centre/right/justified alignment
- Bullet and numbered lists
- Indents
- Hyperlinks
- Tables
Consult Attachments
To upload attachments:
- Click the paperclip icon beside Today’s Notes
- Select a file to upload
Sanitised Content Warning in Consult Notes
For security reasons, MediRecords removes unsafe or unknown content if it is pasted into Today’s Notes or appears in older consult notes.
This may occur if the content was:
- Pasted from external editors (e.g., Microsoft Word)
- Auto‑inserted via an API
- Stored in Notes before this update and contains unsafe formatting
- Copied from a third‑party system
What you may see: Today’s Notes warning
If unsafe content is detected while writing or editing Today’s Notes, you will see this message:
What this means:
- MediRecords has removed or cleaned unsafe content
- Some formatting, links, or images may appear as plain text
- You should briefly check your notes to ensure everything is accurate
Why does this happen:
These updates help ensure the safety of your patient data and prevent harmful content from being introduced into MediRecords. Most users will rarely encounter these messages.
Troubleshooting
A consultation note can’t be reopened or edited
Cause: Once a consultation is finalised, whether by clicking Finish or through Automatic Consultation Finalisation, the original note can no longer be edited.
Resolution: Add an amendment to the finalised note instead of trying to edit the original text.
Escalate when: An amendment cannot be added to a finalised consultation, contact MediRecords Support.
A clinical item doesn’t appear in Today’s Notes
Cause: The item was created from the Patient Dashboard without first entering the Clinical section of the consultation.
Resolution: Add or re-add the item from within the Clinical tab during the consultation so it’s captured in Today’s Notes.
Escalate when: A clinical item added through the Clinical tab still doesn’t appear in Today’s Notes, contact MediRecords Support.
An expected Room doesn’t appear in the Room dropdown
Cause: Rooms must be created under Practice Settings before they’re available to select against a consultation.
Resolution: Create the room under Practice Settings, then it will appear as an option.
Escalate when: A room has already been created under Practice Settings but still doesn’t appear in the dropdown, contact MediRecords Support.
Some pasted content is missing or appears as plain text in a consult note
Cause: MediRecords removes or cleans unsafe or unrecognised content pasted into Today’s Notes, for example from external editors, third-party systems, or auto-inserted via an API, and also cleans older notes that contain unsafe formatting.
Resolution: This is expected behaviour, not a fault. Briefly check the note to confirm everything is accurate, then continue editing normally.
Escalate when: Content in the note appears incorrect or important clinical information seems to be missing after this message appears, contact MediRecords Support.
